Documentation

Provide documents listed below at the school office your student(s) will attend. Missing documents may delay registration completion. 

• Provide proof of residency (Acceptable documents include: Property Tax Statement, Property Deed/Contract, Current Utility Bill, Lease/Rental Agreement or Receipt, or Notarized Letter from the Landlord).
• Other required documents for each student .
  • Proof of legal name/age (example: Birth Certificate, Legal Document, Passport, Visa)
  • Current Immunizations or Exemption
  • Name/Address/Fax number of previous school.
  • Proof of Guardianship / Court Documents (if needed).
